Reimbursement of Medical / Hospitalization Expenses

Reimbursement of hospital & medical expenses incurred due to protracted illness (viz. T. B., Paralysis, Mental disease, Brain Tumor etc).

Major accidents or surgical operations as under

• Angiography ₹ 5,000/-
• Angioplasty ₹ 15,000/-
• Bypass surgery, Cancer, Kidney transplant/brain/spinal surgery ₹ 20,000/-
• Eye / Brain surgeries / major accident / operation ₹ 5,000/-

In cases other than A, B C & D above, reimbursement is (whichever is lower) of

• 50% of the expenses incurred of which original bills are submitted OR
• 50% of difference between amount claimed and settled by employer OR
• ₹ 15,000/- per member

Conditions

• Original bills of approved medical hospitals/institutions of Public and Charitable Trust
• In case of category E (ii) above, copy of certificate of reimbursements and bills paid by employer

Eligibility

• Shareholders above 50 years of age
• Shareholders for minimum 5 years with minimum shareholding of 50 shares
• Reimbursement will be made once in two yea₹
• Shareholder should not be a defaulter as borrower / guarantor while applying for the said scheme

The Scheme enables free medical checkup to

• All members and their spouses
• Amount Of Reimbursement
  
  

  For Shareholders holding 500 or more shares of the Bank.

  • Rs.5000/- maximum or actual expenses incurred whichever is lower.
  • Rs.5000/-maximum or actual expenses incurred whichever is lower for CT Scan subject to recommendation from doctors.

  
  For Shareholders holding less than 500 shares of the Bank.

  • Rs.1500/- maximum or actual expenses incurred whichever is lower.
  • Rs.3000/-maximum or actual expenses incurred whichever is lower for CT Scan subject to recommendation from doctors.

• Shareholders above 50 years of age
• Shareholders for minimum 5 years
• Reimbursement will be made once in two yea₹

Procedure in brief

• Submit original bills / employer's certificate for medical reimbursement
• Payment made directly to centre or reimbursed on the basis of payment receipt

Eligibility Criteria

Eligibility

• Shareholder minimum for 1 year
• 55% criteria will not be applicable in case of Post – graduates having Professional degree viz Doctor, Chartered Accountant, Cost Accountant, MBAs, CFAs or any other professional (degree) qualification

Procedure in brief

• Submit application with attested copies of mark sheets for examination cleared or the certificate of distinction within two months from the date of the results

General Conditions (For all the above schemes)

• Submit all application to MD of the Bank
• Bank has the sole discretion to sanction part / full, or reject decision will be final
• Can avail the scheme once in a co-operative year. (1st April to 31st March)
